The Licensed Practical Nurse (LPN) Ambulatory Care provides direct and indirect patient care under the direction of the Registered Professional Nurse, Nurse Practitioner, Physician’s Assistant, or Physician. The LPN functions as part of the interdisciplinary health care team. May be required to make home visits.

Provides direct and indirect nursing care individualized to the patient’s needs. Collects and records relevant patient health data accuratelyEvaluates health data to identify patient problemsImplements and documents specified interventionsIdentifies deviations from expected patient outcomesAdheres to approved, research-based practice standardsPerforms and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and proceduresMaintains and documents testing reagents/supplies and equipment according to established laboratory proceduresAdvocates for patients to ensure that care preserves and protects the patient’s autonomy, dignity and rights.

Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversityAssures that patients have informed consent signed for indicated proceduresMaintains current licensure, certification, and competencies. Provides proof of current BLS validation to supervisor. Completes required unit credentialing according to departmental policyProvides to supervisor evidence of completion all required annual training and competency validationPractices within the New York State-defined LPN scope of practice 100% of the timeAcquires and maintains current knowledge and skills in his/her area of practice.

Pay TransparencySalary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. Minimum:USD $18.02/Hr.

Maximum:USD $27.03/Hr.
