You’ll need
Licensure
✓Current pharmacist licensure in the State of California.
✓Has BC-ADM (Board Certified - Advanced Diabetes Management) certification, or ability to obtain within three years of hire
Board Certification in Pharmacotherapy (BCPS) or in specialty area is preferred
Certifications
✓BLS certification upon hire
Education
✓Pharm.D. degree from an accredited School of Pharmacy in the USA
Qualifications
✓Completion of a PGY1 and/or PGY2 residency or equivalent practice experience
✓At least three years or equivalent experience as an inpatient pharmacist in a major medical center
✓Passionate about improving the safety, quality, and effectiveness of inpatient diabetes care, particularly insulin use and glycemic management
✓Possesses good interpersonal skills and the ability to communicate complex clinical, pharmacotherapeutic, and operational information clearly to diverse audiences
✓Demonstrates strong clinical judgment and critical-thinking skills, including the ability to evaluate glycemic data and provide individualized, evidence-based recommendations for complex hospitalized patients
✓Has an interest in using electronic health record tools, data analytics, and real-time surveillance to identify glycemic excursions and support timely clinical intervention
✓Demonstrates a commitment to quality and process improvement, outcomes measurement, and translating evidence into sustainable clinical practice
✓Exhibits excellent verbal and written communication skills
✓Demonstrates ability to prioritize projects to meet goals
