Description

Job OverviewWe are seeking a dynamic physical therapist to join our outpatient flexible workforce team! The physical therapist will serve pediatric through adult patients across multiple outpatient, multi-specialty therapy clinics in the greater Twin Cities metro area. Preferred candidate will have experience in providing interventions for Ehlers-Danlos, vestibular and concussion related impairments.

Where training and experience is limited, Fairview offers mentoring and development opportunities for professional growth. In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.

FTE: 1.0, (80 hours per pay period).

Monday - Friday, generally 8:00am - 4:30pm; shift can vary +/- 30 minutes, depending on the needs of the clinic being covered. Provide coverage at various locations across the Twin Cities metro area; including, Blaine, Burnsville, Eagan, Edina, Maple Grove, Maplewood, Minneapolis, New Hope, St. Paul, and Woodbury.

Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

ResponsibilitiesThe physical therapist partners with the patient and family while performing the Evaluation/Re-evaluationGathers pertinent dataPerforms examinationDetermines treatment diagnosisDevelops an individualized plan of care through collaboration with the patient and familyAnalyzes data to identify needs and potential risksDetermines prognosis for recoveryEstablishes individualized goals with patient and/or caregiverIndividualizes plan of care considering developmental level and cultural differencesImplements and modifies plan of care based on reassessment and patient responseCoordinates care and communicates effectively with interdisciplinary team for all aspects of patient careEstablishes appropriate discharge planPartners with patients and families to provide skilled interventionImplements plan of careAlters treatment/plan of care to reflect change in patient status and response to treatmentAssesses patient and/or caregiver learning needs and provides appropriate educationRequired QualificationsPhysical Therapy License in the state of Minnesota or temporary license in the state of MinnesotaBasic Life Support (American Heart Association or Red Cross)
