Day shift | Part-time How you’ll make an impact in this role Create and oversee diabetes education programs that align with ADA standards, focusing on prevention, management, and patient empowerment. Work with physicians, nurses, dietitians, and other team members to create and implement individualized care plans for diabetes patients. Educate patients and families on self-management strategies, including lifestyle modifications, medication, and self-monitoring techniques.
Track patient progress, assess program effectiveness, and adjust strategies to ensure optimal diabetes management and adherence to ADA guidelines. What minimum requirements you’ll need Licensure / Certification / Registration: Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required. Advanced Life Support preferred.
American Heart Association or American Red Cross accepted. BLS Provider preferred. American Heart Association or American Red Cross accepted.
Educator specializing in Diabetes Management credentialed from the American Heart Association (AHA) preferred.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of Hire.
