Develop and implement individualized care plans based on assessment findings and patient needs. Coordinate multidisciplinary care by facilitating collaboration among healthcare professionals, including physicians, nurses, social workers, and therapists. Monitor patient progress, regularly review, and adjust care plans based on patient responses and outcomes.
Communicate effectively with patients, families, and the healthcare team to ensure everyone is informed and involved in the care process. Document patient information accurately, including assessments, care plans, interventions, and outcomes in the patient's medical record. Knowledge, Skills, and Abilities:Skills required to work in the emergency department as defined by unitMust be able to use a high level of abstract reasoning to assess patients, evaluate interventions, and revise the plan of care according to patient outcomesBasic computer skills – must be able to navigate Microsoft Windows-based programsDemonstrated proficiency in acute-care nursing, knowledge, and skillsValid American Heart Association (AHA) Basic Life Support (BLS) certification, and valid AHA Advanced life support certification (ACLS, PALS, NRP) per specialty/unit requirementsInternship and/or preceptorship dependent upon nursing experienceUnit-specific competencies met per orientation pathwayThe majority of employees will require a minimum of 12 months after orientation completed to achieve proficiencyEducation:Associate's of Nursing [Required]Bachelor's of Nursing [Preferred]Work
