- Department and Program Summary The MaineHealth Preble Street Learning Collaborative (MH PSLC) is a low-barrier clinic that addresses the needs and care of people experiencing homelessness through education, outreach, and walk-in medical services, case management, and other community resources. In 2024, the MH PSLC launched the CONNECT Outreach Van — a grant-funded mobile outreach program that delivers trauma-informed treatment for psychiatric and substance use disorders to people with unstable housing in Greater Portland. The core CONNECT team includes a Clinical Director/Psychiatric Mental Health Nurse Practitioner (PMHNP), Medical Outreach Case Manager (MOCM), Rapid Rehousing Services Caseworker (RRHS), and a program coordinator. The Registered Nurse II position will provide care services to CONNECT patients and offers the opportunity to join a close and supportive interprofessional team. This position is part-time with benefits, covering 0.6 FTE (e.g. 3 days per week). Helpful Professional Experience Prior work experience in the following areas would be beneficial for this position:
- Training in and/or experience with Trauma-Informed Care
- Experience working in low-barrier settings
- Experience working with people experiencing homelessness
- Knowledge of substance use disorders and psychiatric disorders
- Training in and/or experience with wound care Scope of Work The following duties reflect the position's
but do not restrict the assignment of additional tasks. 1. CONNECT Patient Care Provide trauma-informed nursing care to CONNECT patients in collaboration with the PMHNP and MOCM. Care delivery will take place in both community settings — including on the street, at encampment sites, shelters, and Housing First sites — as well as at the MH PSLC outpatient clinic.
Outreach visits may be conducted with other CONNECT team members or individually. Responsibilities include but are not limited to: patient assessment and triage, wound care, vital signs monitoring, health education, medication support, care coordination, and implementation of individualized care plans, including support with psychiatric and substance use treatment plans. The RN II will also provide ongoing nursing support to patients who obtain housing through the CONNECT Rapid Rehousing Services, including more intensive short-term follow-up and wound care management when indicated.
The CONNECT patient panel typically includes approximately 50 active patients, with about 15 patients obtaining housing annually. 2. CONNECT Team Meetings & Program Support Participate in daily huddles, monthly team meetings, and case review discussions to support coordinated care planning and patient outreach efforts. Collaborate with the Project Director and Program Evaluator to support grant reporting and evaluation activities. 3.
Clinical Administrative Management Complete timely and accurate documentation of patient encounters in the electronic health record. Maintain required clinical and regulatory documentation, including authorizations, consent forms, and treatment-related records. Support care coordination activities such as referrals, follow-up communication, medication-related documentation, and tracking of patient care needs.
The MH Registered Nurse - Ambulatory Care role provides high quality care to patients and families calling or presenting for care through assessment, planning, implementation, and evaluation, and performs a wide variety of patient care services to assist physicians/advanced practice practitioners in delivering excellent care to patients of the practice. MH RN II: This level is competent in their clinical role and has demonstrated the ability to care for most patients assigned.
Though not able to care for every patient type within their area of hire, demonstrates how to obtain the appropriate resources to develop required skills and knowledge over time. This level included automatic advancement after 1 year as a RN I – must be in good standing and meets RN I requirements. Required Minimum Knowledge, Skills, and Abilities (KSAs)
Graduate of an accredited School of Nursing required; BSN preferred. License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required. Current BLS certification required or must obtain within 30 days of start date.
One year of recent RN experience required. Automatic advancement from RN I level if all onboarding requirements are complete, including the New Grad Residency (where available). The base range is an estimate and does not reflect the full value of our total compensation package. The pay range for this position is $34.35 to $46.46 per hour. Compensation for part-time positions is determined on a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
