Registered Nurse (RN) – Acute Care Case Manager Facilities: Petaluma Valley Hospital and Healdsburg Hospital Shift and Schedule Day

8:00 AM – 4:30 PM 5 days per week, 8-hour shifts. Every other weekend required. Must float between both facilities, approximately 30 minutes apart.

Weekly schedule may vary between locations depending on staffing and PTO. Required Experience and Skills Minimum 2 years of experience listed in job qualifications. At least 1 year of acute-care hospital Case Management experience is a hard requirement.

Epic experience required — no exceptions. InterQual criteria experience required. Active Compact RN license must be in hand at submission.

Travel experience required. Experience with care coordination, admission criteria, discharge planning, and CMS guidelines. Patient and Unit Details Adult and geriatric patients.
