United Health Services (UHS) is seeking a dedicated Pharmacist to join our inpatient pharmacy team. The Pharmacist will ensure the safe and accurate administration of medications by verifying the clinical appropriateness of prescribed treatments and overseeing proper delivery methods, thereby promoting patient safety and optimizing therapeutic outcomes.
Review medication orders for appropriateness (dose, route, frequency, indication, interactions, allergies)Verify and dispense physician-ordered medications in accordance with hospital protocolsPerform medication reconciliation upon patient admission, transfer, and dischargeMonitor patient therapy outcomes and adjust or recommend changes to optimize treatmentProvide drug information to healthcare providers, patients, and caregiversEvaluate and prevent adverse drug events and medication errorsEnsure compliance with hospital formulary and therapeutic guidelinesMaintain inventory control of medications, including controlled substancesDocument all activities in the electronic health record (Epic)Ensure compliance with state and federal regulationsPrecept pharmacy students and residents in clinical and operational settingsPosition QualificationsMinimum Required:A Bachelor’s of Science Degree (B.
S.) or Pharm. D. in Pharmacy from an accredited college or universityLicensed as a Pharmacist in New York State or NYS internship permitPreferred:One year experience as a pharmacist
