- Guaranteed Hours Per week: 36-48 (minimum of 36, up to 48) *GSA hours are not guaranteed the week of Orientation. Traveler is guaranteed set hours per week, except during holidays where unit is closed*
- Primary Location: Bellin Home Health (1920 Libal St, Green Bay) **Starting at the Home Health office and covering a 9 county radius in NE WI (average travel of 75-100 miles per day, 4-6 visits per day with travel time included in shift length)**

Current WI RN license or Compact. AHA BLS. • Required
Graduate from an accredited school of nursing (BSN preferred)• Minimum
- 2-3 years of ACUTE home health and Hospice experience, acute experience absolutely. REQUIRED.*They need they see acute patients that are currently in a home health setting. It is a very different/independent environment*
- EPIC Experience REQUIRED. Point of care documentation experience REQUIRED. OASIS Experience STRONGLY preferred by not required.
- Desired Specific Skillset for an ideal candidate: o Comfort with hands on skills such as IV site care, wound care, wound vac, ostomies, etc. o Comfort in all home types (no special requests can be made like no pets, no smoking, etc)o Up to 50 pounds of lifting, pushing, and pulling during patient care may be required. (Examples: pushing a patient in a wheelchair or repositioning a patient)

- Unit Overview: We care for a wide variety of patients. Ages range from newborn to our senior population. We provide services to patients following an illness, new diagnosis, injury, or surgery; with the goal of assisting patients to return to their prior level of function or better! Our clinical team includes a multidisciplinary team comprised of nursing, physical/occupational/speech therapy, social work, and CNA staff.
- Scrub Color Required: Solid color scrubs or business casual dress is fine
- Orientation: Length of time varies depending on role/experience. Additional things to note: o Staff is mindful of inclement weather and will steer visits away from effected areas when possible, very protective and mindful of staff safety o Solid color scrubs or business casual dress is fine o Provided to team members: Laptop, corporate phone, clinical bag with vital equipment/PPEo We utilize advanced technology through our remote patient monitoring program to closely monitor patient vital signs and other key clinical information including listening remotely to heart and lung sounds, capturing a 1-lead electrocardiogram, or conducting a video visitBilling: Sunday - Saturday8hr unbillable orientation *GSA hours are not guaranteed the week of Orientation.
